- Follow‑up medication management: $100
- Payment is due at the time of service.
- A card may be kept on file for your convenience.
- Superbills are available for out‑of‑network reimbursement.
No‑Show & Late Cancellation
- We require 24 hours notice for cancellations or rescheduling.
- No‑Show Fee: $55
- Late Cancellation Fee: $55
- Fees are automatically charged to the card on file.
- Repeated missed appointments may result in discharge from the practice.
Telehealth Consent
By participating in our care, patients acknowledge and agree to:
- The use of secure telehealth technology.
- Participating from a private, safe location.
- Understanding that emergencies require calling 911.
- Electronic communication may be used for care.
- Telehealth has both risks and benefits.
Financial Agreement
By receiving services, patients agree to:
- Be fully financially responsible for all charges incurred.
- Pay all fees at the time of service.
